California sr-1 form

If you have been involved in a traffic accident in California, the law requires that if there was an injury, death or property damage (in excess of $500 or $750) the accident must be reported to the Department of Motor Vehicles (DMV) within 10 days of the incident.  It is the responsibility of every driver (regardless of fault) who has been involved in an auto accident meeting the requirements outlined below to complete and submit an SR 1 form with the DMV.  A police report, or any other submission by any other party does not satisfy your requirement of completing and SR 1 form if you have been involved in an accident.

  • Accidents occurring on December 31, 2002 or earlier – Excess Property Damage of $500.00
  • Accidents occurring on January 1, 2003 or after – Excess Property Damage of $750.00
